Insurance, paying via insurance

I am registered with some insurance companies, including AXA PPP, CIGNA, WPA and Simply Health. I am also covered by some BUPA Cashplan policies.

My details for insurance purposes:

Claire Callaghan B.Pty MSc Chartered Physiotherapist

CSP no: 073174

HPC no: PH51958

If you  wish to see me for Physiotherapy and would like to claim your treatment through private medical insurance, you will need to:

1) Contact your insurer prior to treatment. They will let you know if I am on their register. If they pre authorise treatment they will give you an authorisation code.

Please contact me directly should you need my provider number for your insurance company. I have a separate provider number for each company.

2)In order for me to invoice the insurance company directly, I will need your membership number and authorisation code.

3) Please note, some policies will have an excess, which you will need to pay. Most companies pay my full fee.

4) In the case of AXA PPP and their subsidiaries (e.g. Exeter Health), there is a limit of £35 per session.

This sum is based on a 30 minute treatment session, which is shorter than my usual session. If you are covered by AXA PPP or their subsidiaries, treatment sessions will be limited to 30 mins. If you would like to have a longer session as usual (50 mins) that is possible, but you would be liable for the £20 gap on each session.

5) Most insurance companies have a limit on sessions. Please forward any paperwork which needs to be done to enable the sessions to take place. Should this be required, your insurer will send it to you.
