If you wish to see me for Physiotherapy and would like to claim your treatment through private medical insurance, you will need to:
1) Contact your insurer prior to treatment. They will let you know if I am on their register. If they pre authorise treatment they will give you an authorisation code. Please contact me directly should you need my provider number for your insurance company. I have a separate provider number for each company.
2) Provide me with
-
your insurer's name
-
your insurance membership number
-
authorisation code
-
any excess on the policy
-
the number of authorised sessions
AXA PPP and their subsidiaries
Most companies pay my full fee. In the case of AXA PPP and their subsidiaries (e.g. Exeter Health), there is a limit of £50 for the initial session and £40 for follow up sessions. Sessions under these policies are 30 minutes duration. For further information, see here.
